Patient Information
In order to be seen by a vascular surgeon, you need to be referred by your family doctor or another physician.
- Please bring your Health Card and Medications with you (you may bring bottles or a list including name, strength and how many times a day you take the medication).
- If you have been seen by a Cardiologist (Heart Doctor) in the past and he/she did not make this referral, please call their office to have copies of your records faxed to our office prior to your appointment.
- Peterborough Vascular Surgery Medical Questionnaire — download and print, fill out and bring with you.
If you are unable to attend your appointment and need to reschedule at a later date, please call our office at 705-743-6760.
We value your time and we do our best to keep your wait in the reception area to a minimum.
We make every effort to schedule appointments so the needs and concerns of each and every patient are met.
Please be aware that emergencies occasionally require us to reschedule your appointment.
If you are having a vascular ultrasound at the Peterborough Vascular Lab, your appointment with the vascular surgeon is typically scheduled one hour after your test appointment time in order for your results to be available for the vascular surgeon.
If your ultrasound test requires you to fast, please bring along a snack and a drink as you will be able to eat and drink after your test. If you are diabetic, it is always a good idea to have a snack and/or drink with you just in case you do need to wait for your appointment.
The Peterborough Vascular Surgery website was created as an information site only. Requests emailed through the website for the purpose of medical advice, follow-ups and or booking appointments will not be answered. It is strongly recommended to phone the physician’s office directly should any medical questions and or concerns arise.
